Life-sustaining therapy and the Disability Tax Credit

Therapy that supports a vital function is its own DTC category. What counts, the old 14 hour test, the 2022 change for type 1 diabetes, and how to document it.

Can life-sustaining therapy qualify for the Disability Tax Credit?

Yes. Life-sustaining therapy is its own category under the Disability Tax Credit, covering therapy needed to support a vital function, like insulin therapy for diabetes. Since 2022, people with type 1 diabetes are generally considered to meet this requirement. The therapy must be ongoing and certified by a medical practitioner on Form T2201.

Most DTC categories are named for a part of daily life: walking, seeing, hearing, speaking. Life sustaining therapy is different. It is named for the treatment itself, the recurring therapy a person needs to keep a vital function working. For the families inside this category, life runs on a schedule set by medicine. Here is how the CRA sees it, and what changed recently in their favour.

What life-sustaining therapy means

The category covers therapy that is needed to support a vital function. The classic example is insulin therapy: for a person who needs it, insulin is not optional medication, it is what keeps their body working safely. Dialysis and certain other recurring, essential therapies sit in the same family. The common thread is necessity. Skip the therapy, and health is at risk in a way that goes far beyond discomfort.

This category exists separately from the basic activities of daily living because the burden it measures is different. It is not only about what the body cannot do. It is about the hours of care the body requires, every week, without holidays, to stay safe. Families living it rarely need that explained. Assessors sometimes do.

The old time test, and why families argued about minutes

Historically, this category was measured in time. The general test looked at whether therapy took an average of at least 14 hours per week, at least three times per week. That turned applications into accounting exercises. Families kept logs in notebooks and on phones, counting dose calculations, monitoring, and adjustments, and many were worn down by the counting itself and gave up.

I describe that history in my guide to diabetes and the DTC, because diabetes families carried the heaviest share of it. If older advice told you to start a minute by minute diary before even thinking of applying, you were hearing the old rules. The landscape has shifted, and it is worth a fresh look.

What changed for type 1 diabetes in 2022

Since 2022, people with type 1 diabetes are generally considered to meet the life sustaining therapy requirement for the Disability Tax Credit. The government recognized what families had been saying for years: managing type 1 diabetes with insulin is, by its nature, life sustaining therapy. The daily monitoring, dosing, and adjusting is not a small extra task layered on life. It is the therapy, and it never takes a day off.

Generally considered is still not a guarantee, and the application still runs through Form T2201 with a practitioner’s certification. But a family that was discouraged under the old hours counting era should not carry that discouragement into today’s rules.

Other therapies, and documenting the load

Insulin is the best known example, not the only one. Where a recurring therapy supports a vital function and consumes a significant part of life, the category may apply, and the practitioner’s description carries the claim: what the therapy involves, how often it happens, what happens without it, and how long the pattern has been in place. Effects must be prolonged, lasting or expected to last at least 12 continuous months, as with every DTC category.

If the therapy in your life is measured in sessions and schedules rather than symptoms, this is your category to investigate. The free DTC eligibility checker covers it, and my companion guide on bowel and bladder functions shows how a neighbouring category is written. Bring your real schedule to the conversation. In this category, the schedule is the evidence.

Common questions

What counts as life-sustaining therapy for the DTC?

Therapy needed to support a vital function. Insulin therapy is the classic example. The therapy must be essential and ongoing, and a medical practitioner certifies it on Form T2201.

Do I still need to log 14 hours a week?

The historical test counted an average of at least 14 hours per week of therapy. Rules have evolved, and since 2022 people with type 1 diabetes are generally considered to meet the requirement. Check the current criteria for your therapy before applying.

Can type 2 diabetes qualify under this category?

It can, depending on the therapy involved and its effects. Insulin therapy is the key route. A diagnosis alone is not the test; the therapy’s necessity and burden are what the CRA assesses.

Who certifies life-sustaining therapy on the T2201?

A medical doctor can certify this category. The practitioner describes the therapy, its frequency, and why it is needed to support a vital function.
